Dispute and block letter for an identity theft account
Use this when an account on your report was opened by someone else using your information.
When to use this template
- An account, inquiry, or address you didn't create appears on your report.
- You've received bills or notices for accounts you never opened.
Sample letter
Replace everything in [brackets], and send one letter to each bureau that shows the error.
